The American Heart Association’s virtual Regional Hypertension Summit brings together health care organizations, medical practices, and health-focused organizations from across New York State to collectively address blood pressure control and recognize Target: BP™ achievements. Target: BP is a national initiative formed by the American Heart Association (AHA) and the American Medical Association (AMA) in response to the high prevalence of uncontrolled blood pressure.
Sessions focus on the AHA/ACC hypertension guidelines and evidence-based blood pressure activities inspired by the MAP framework (Measure accurately. Act rapidly. Partner with patients.). The Summit features local and national experts and create a network of learning amongst providers and community-based organizations that have a role in hypertension management.
Audience: Clinical and population health representatives from health systems, FQHCs, clinics, home health agencies, including PCPs, CHWs, pharmacists, dieticians, and other team members. Community-based representatives with a role in the treatment and/or management of hypertension are encouraged to attend as well.
